There were fewer incidents when the players could police themselves. Take fighting out of the game and the only way they can attempt to police a situation is taking a run at someone which leads to more concussion problems.
About 30 years ago if someone pulled a p****y move like Malkin's attempted elbow that player would have promptly gotten his ass kicked. What happens in a game is all about emotion. The threat of an ass kicking will stir up much more immediate emotion than the threat that maybe if the ref sees it I will get suspended.
