Check out the official slider thread. There are some camera angles in there. Nolefan has one posted in his sliders that is pretty good. I tweaked it a tiny bit for my liking but it's good starting point. I think it's on page 11 of that thread.

I use standard with pass zoom out and QB pivot on. The only time it bugs me is when you are inside the 5. It won't zoom out at all to see your receivers.

What up infamous. This is ya boy supersigma2of21 from MM. I use Custom. You can turn on Pass Play Zoom Out if you like. It makes it easier to see the WRs.
