Perplexing Draft Pick Penalty

In a multiplayer league, the commissioner just clicked "Begin Amateur Draft". Upon viewing the draft order, the commissioner notices pick 3-30 has "xxx" where the team abbreviation should be - indicating that team has been penalized pick #3-30 for some reason.

The problem is, the commissioner did not click on pick 3-30, and until that moment, no one who may have had access had an an opportunity to do so as the picks are not revealed until the commissioner clicks the "Begin Amateur Draft" button.

The commissioner did not save the stage, so rebooted and reloaded and the same result occurs, pick 3-30 is listed as "xxx".

The commissioner did not save the stage in hope there is some repair to this.

Two questions.

1) Any idea how this happened?

2) Anyone know how to fix it?

The team in question was over the salary cap at the conclusion of FA-I. It's a standard penalty and should be in the help file.
