If I want to play three IM's in a 4-4-2 is it better to put one IM in a wing slot and play towards the middle? Or would taking one of the forward slots and making it an "extra midfielder" and then taking one of the wing slots and making it an "extra forward" be better? I can get better ratings putting the latter lineup together but wondered if moving too many players around would lessen the effect.

Oh and do I lose a half week of training by playing an IM towards the middle in a wing slot?

I think I just answered my own question. To maximize midfield I want to to three IM's and then play my wing towards the middle correct?

The guy in the wing spot is not an IM toward the middle...he's a winger towards middle.
He does not get trained as an IM, he is a wing. He gets 50% training if you're a PM trainer.

If you're working to max midfield with a 4-4-2, then yes, play 3 IM's (the extra IM coming from a forward spot), play one wing as a wing to middle, and the other wing repositioned as a forward.

or wing as extra CD and a cd to extra IM.
