HTML Code for Voting

I am trying to put a form on our league page to vote for League MVP. Can anyone help me out with the code to have the vote, ask for GM of Team, their email address, and then has 3 blank text fields to vote in and a submit/reset button, and when they submit to send it to my email address? I looked on Bravenet, and it gave me an example, but I can't get the format to work correctly from the example they provide.

I assume you're talking about NAFL, gottimd. Honestly, I think that voting on the forum will work better because it's easier. You'll get more people to participate that way, I'd bet.

:D Yes I am, but I've automated everything through excel so it takes me a few minutes to update financials, whereas others it takes hours, so I have a wee bit of free time on my hands.
