Closest I know of is the North Plainsfield Plague, they won two and lost the third championship game by 2.

WrongWay did it four times in a row in the FastFL...but that is WrongWay and the FastFL, so it really doesn't count.

The DFL Titans are in the playoffs again, so it's possible that we could. Somehow. So I was wondering if anyone else had done it.
