OK, here's the scenario, its just past midseason, and you are vying for the division title. You are currently a game behind the division leader, and a game up on the other WC contenders. You are playing the division leaders for the first time this season at home, but your starting QB is probable with a sprained knee.
What do you do?

To my mind, there are a few other factors I would need to consider here:
- age of starter - if the starter is a 15th-year veteran who isn't going to be back next year, I'm much more likely to play him than if I have a talented young QB as my starter
- quality of starter compared to the backup - if there's a big drop-off between #1 and #2, I may be more likely to play the starter (I use 'may' here because a big difference might mean that losing our starter for the year would in all likelyhood mean that we won't be good enough to be a playoff team...) - on the other hand, if I have a capable backup, I won't take the risk on the starter.
- type of offence/quality of supporting cast - if I have an offence that doesn't rely too heavily on the quarterback (e.g. pounding running game with big offensive line, or top-quality receivers who could make Ryan Leaf look like an All-Pro), I'll be more likely to sit the starter.
In the end, it is a judgement call - the importance of the game means that I'd be more likely to play the injured QB, but playing a guy with a sprained knee can have career-threatening consequences if he gets another hit on that knee...

Always play guys with concussions. Nothing bad has ever happened to players with concussions.

It actually boosts their performance since there is no worry about the player over-thinking his assignments.

I play all good starters that are probable with anything but a concussion. In big game, I would even play a questionable with sprained knee. I used to be scared of the sprained knee, but I've noticed that 2K7 says it's only "moderate" risk of reinjury. That's good enough for me.

True story, back in eighth grade I had a serious concussion. But, having had several concussions growing up, I kept playing football in the backyard. I had my best game ever as a QB leading back our side to a dramatic win. I can still vividly remember that part of the game.

For the record, I started him and he led us back from a 12-0 deficit to a 25-22 win. He even mounted 3 4th quarter drives, 2 in the last five minutes (although one ended in a WR fumble).
