I think I have found a decent gameplan mix, however, my YPC is always very low, even though I am winning games. I have noticed this with different teams in different leagues.
Does anyone want to share a couple keys to YPC success, specifically in gameplanning, not so much personnel, that goes w/o saying you need a RB with the right skills and OL-men that are good blockers. I think I have those things for the most part but I'm wondering about situations and formations, etc that might be the root cause.

Just off the top of my head, I would say that when you are winning your YPC will drop naturally because the CPU default is to run more when ahead, and the CPU defense becomes more run oriented as well.
So if you are going into the 4th quarter up by a touchdown or so and have not adjsuted your situational gameplan otherwise the you will begin to pound the ball; into a defense that is looking for a run so you will have less success as far as YPC goes.
Solevision and the game logs will give you a good idea if this is really what is happening.
Related to this I would really like to see a success percentage (Similar to Football Outsiders) for running backs because it would give us a better idea of how they are truly performing. As of right now it gets cumbersome because you can only evaluate based on 3rd down coversions; first down per attempt ratios (that you have to calculate yourself); 10-yard plus gains per attempt ratio (again you have to calculate this yourself).
EDIT: It gets even more cumbersome in an attempt to evaluate your own players against free agents, because you have to set up a spread sheet and punch this stuff in and then try to weight the importance of each ratio. I don't mind the scout error, but I think we should have more in-depth statistical evaluation tools to compensate...without spending time designing spreadsheets (Which is way to similar to actual work as far as I am concerned.)
