What are the settings used for salary cap? Should I use the default or is there a more realistic setting that most people are using to simulate the NFL?

I think the most realistic and best setting is probably something like 70-99 -- higher cap increases seem to help fend off AI teams getting into too much cap trouble, which can cause a pretty meaningful background disturbance.

I had a related question about historical leagues. I remember reading long ago, but can't find it, people's suggestions for doing historical salary caps. I'm playing a league that started at a very low cap, and I'm not sure what to set the increase value at to keep it reasonable. Any suggestions?

I think a reasonable annual cap increase is about 5-8% of the total salary cap. So, if your cap is set to 30 million, for example, maybe a range like 15-25 would make sense.
