Printer Issue

I have a relatively new Epson printer, it's an all in one machine - scan, copy, print. I recently put a new black ink cartridge in, followed the on screen step by step instructions, but every 3rd or 4th line of text that prints, is missing the top half. It's really bizarre and I've never seen anything like this.

I've tried re-replacing the ink, cleaned the cartridge heads, printed test pages, nothing is helping. Before I pull the cartridge and try to return it as defective, I figured I'd check here and see if anyone has run into this before...
