I contacted Solesismic, however I thought I'd see if anyone else has noticed this occurring.
My C is excellent and I decided not to carry a backup. He got 'Turf Toe' and was listed as Doubtful. Well its only one of ten, and it said he could maintain a decent level of performance. Plus I decided not to carry a bC this season. So I played the games. Since we won four out of five, I never really looked at the stats until game 8. I checked the blocker section and saw only four names. Odd that. Even odder my LG had exactly twice the number of plays as the other linemen. I checked the C and he had not appeared in any game after the third of the year. Looking at the team stats, only four linemen had started every game, the C had 3 starts and no one else had started. Plus the LG has been on the field for about 1000 plays and the other three guys 560. So there's that.
The game simply isn't putting him in and using the LG at both spots instead of a backup G or T.

I started up a new career as the Browns. In game 11, the LG got a concussion and Joe Thomas began playing two spots. He finished with more offensive plays than the team had by 99 to 64.
I'll be putting FOF8 on hold until this is fixed.

I wonder what causes this. It doesn't happen every time. Today in my CyFL game, my starting LT was injured in the 2nd quarter and missed the rest of the game:
1-10-COL50 (2Q: 05:50) Jay Bocaling ran inside the right tackle for 14 yards. Tackled by S Dakota Talley. Key block delivered by Maximus Switzer. PENALTY: DE Skip Gonzalez of Columbus (OH) was called for Too Many Men on the Field. The penalty was declined. San Francisco's Zack Holman was hurt on the play.

It didn't even use Carlisle, an OT. It used the exact guy I would have wanted, the OG with full positional experience at LT:

And to verify, my other linemen and my QB were all in for 26 run plays and 41 pass plays. Holman had 12 run plays and 16 pass plays, while Bailey had 14 run plays and 25 pass plays. Perfect.
For reference (in case these matter) my "emphasize exact position match" is set to 7 for offense, and use of injured players is set to 9. Here are the box and log:

I also wonder if this issue is related to OL who get a 'concern' in the Endurance box but still always are on the field for every play. This also happens routinely for me.
My impression is that OL aren't being subbed well at all in FOF8.

Fwiw, I reported this bug a week ago.
Edit: to clarify, I reported the issue where the same player played at 2 positions on the OL. It has happened quite a lot in IHOF since we moved to FOF8, at some point 1 player played 2 spots for 6 full games.

My impression is that OL aren't being subbed well at all in FOF8.As far as I can tell from my little research so far towards this very annoying bug: OL players are not subbed out in FOF8, except for some rare late game situations. They are not substituted based on their endurance, but only based on injuries.
When a player gets hurt (or was hurt going into the game and randomly determined to be too much hurt to play after all) said player get replaced by a random other OL player, which could be one of the 4 other starters. In the case of turf toe, you could get very lucky (or even trick the game) and have your best OL guy play at 2 positions for many games.
As a side effect, it's likely at least one of the 10 All-League OL guys is actually a beneficiary of this bug and played additional snaps for several games.
At least that's what the cosmetic part says, we can only assume (and should assume) the 5 listed OL in the game log were on the field for that play.

Now this is getting weirder. The QB is going in to play RT and allowing sacks:

On further review, it appears to always happen with 113 in all its flavors.

The pre-8.0d OL substitution bug was happening on a weekly base in our MP league, but I don't see anything of that like yet in the first 5 weeks of our first 8.0d season.

I sent them the game log, so they have it all.
