I usually swear allegiance to the King of my choice and take on the tasks he gives me as a result, that's how you get a fiefdom. I don't think that's critical though, you can just build an army and conquer castles yourself as well.
As to factions, I have yet to see it make a difference among the foot soldiers, but any "companions" you pick up can have differences that lead to little events you have to settle and possibly drive one or more of them out of the party. I've also had it affect morale if I invite prisoners into the party.
