The PS4 and X1 have been out 16 months. 2+ years is usually when you start seeing better games as they ramp down support of the previous systems, though it does seem like the PS3 and 360 have a bit more support as a last gen system compared to previous iterations. This will likely be the last year for full support of those consoles.
Failure rates are definitely down from last generation, though I'd think they'd have to really screw up to beat last gen's failure rates.
