Been there done that in the heavy painkiller days here.
My child, however, adds a wrinkle to answer some of your question.
Legit patient A has a 'script for 1 pill per day. BUT he really only takes 1/4th of a pill per day per discussion with his doctor. The 'script is never revised however ... so he refills once per month but only needs 1/4th of that, the other 3/4th ends up on the street.
Combine that with people knowing which pharmacy might not be so circumspect in their handling of refills and there's a fair supply for the street.
