I think he either alters his brain, or somehow excises a small part of their brain to inject. The trauma would kill those who couldn't heal themselves.

Or he basically slices open someone's skull and exposes their brain, which leads to death. Which is what he does, apparently. Either that, or he loves carving out peoples' foreheads.
