When a cow is in heat, her activity will be greatly increased compared to her baseline. Basically, if there is no bull around, she starts engaging in homosexual acts with females. They have special algorithms using accelerometer technology to match the type of movement with being in heat.
On the health end of things, there are also algorithms associated with eating. A pretty simple rule of thumb, if a cow doesn't eat on a regular schedule she's sick or something else is wrong.
