All righty -- the potion is Metamorph. It lets you change forms for a day. You use it in the morning, then you're informed what forms you can take that day, and what their effects are. Worth 150 gold.

I'm not sure which is which, but here are the other two:

Manual of Weapon Expertise -- Disappears after being read once. It lets you learn how to use all exotic weapons. Only Paladins or Warriors can read it. Worth 200 gold. This is probably the book.

Protection from Magic -- This is a combat protection, that protects you from all damage dealt by magic. Worth 150 gold. This is probably the scroll.

The room I was in had a giant spider and four mushrooms. The spider nearly killed me, but I managed to escape and get back where I was before movement.
