Can additional users be added to a pre-existing online dynasty? if so how and when?

Also what happens if a user gets fired?

the commissioner can add/remove players at any time.

go to Online Dynasty Setup, User Teams & Coaches menu. on 360, "X" adds another user, "RB" removes a user.

nothing, you can't get fired in an online dynasty.
