I think I discovered the issue last year.
As the commish is updating the file and advancing the week, the other players are out of the dynasty. But when the prompt comes up to save the file, other players can re-enter.
DON'T RE-ENTER AT THAT POINT!
If players wait to re-enter the dynasty until after everything is completed and the next week's stuff has appeared on the commish's screen, problems can be avoided.
