For me, the biggest difference is in the awareness. But you also have to consider that if you have a 5* recruit as your backup, he probably won't be too happy about having to ride the bench (and could lead to a possible transfer). Not to mention, if you are a losing team, it might be best to aquatinted with your QB of the future.

It has to be incredibly low, basically my starting qb was done for the year and with Noone else really, I started my te who started as a qb in real life. Things did not go well. Lol

I can not confirm this, pure speculation at this point, but I have heard catching is also affected by you QB's awareness. Seems to be true in what I have seen so far.
