Why is it when I dive with some players, especially the QB, they fumble the ball! They are completely untouched, but can't hang on to the ball during a dive. Happens all the time when I'm doing a QB sneak and I don't want my QB to get crushed, so I dive and at least half the time, he will fumble.

Have your QB slide instead of dive.

Press the protect-ball button instead of the dive button if you want to protect the ball. Also, setting your strategies to "Conservative" for "Big Run" also helps.
