Xbox Unreadable Error

On my Xbox 360 slim, I will be in mid-game and the disc is unreadable error will pop up. However, the game is actually still going on in the background. This only happens during games and not at like the main menu, recruiting, etc. When it goes back to the dashboard, it reads NCAA just fine. I know this isn't really a NCAA problem, but has anyone experienced this or can help me out? It would be greatly appreciated.

take the hard drive out, and play the game off the disc too see if you get any errors. If you don't get unreadable errors than its the hard drive, I had an similar issue but it was caused by a crappy external hard drive on a xbox 360 slim
