Career mode settings question

Thanks in advance for anyone that can point me in the right direction. I feel dumb to have to ask this:
18 games into a Career Mode, and not a single injury (human or CPU). Sliders were default for first 12 games or so, last six I bumped them up, but still no injuries.
In the main menu setting (outside of Career Mode) I can see that injuries are turned on. However, within my Career Mode I can not find those same settings (injury, offside, handball, etc.).
What am I missing here?

In career mode, injuries are a slider. If you in settings, find the sliders, and scroll down a bit.

Offsides, and handball are only turned on outside of career mode, in the games main settings.

I do 80-90 Frequency and between 30-40 Severity. I lower severity when I get 2 injuries out for longer than one week.
