But I would love to see it in UFC 3. I'm probably wrong, but first fighter I saw doing this a lot was Dillashaw.

They kind of duck down and charge forward while shifting upper body weight. In this gif Cody does it first, and then TJ does something similar (but different).

Could be very common in fights, I just first noticed this from Dillashaw back before he was champ.

Even with the multiple upper body shifts they use sometimes? Takedown fakes in UFC 2 weren't very satisfying.

They are trying to bait each other to commit so the other can counter.. Mayweather and McGregor both do this: by leaning your head forward over your lead foot you give your opponent the illusion that you're closer to them than you are. Then when they lunge in, you can move back and counter but obviously, this is more of a chess match because they know each other very well so they are aware of each others strengths/weaknesses.

I don't think I've seen Conor use forward locomotion with it.
