yeah that is one factor that contributes to the option game, but it always doesn't have to be an outside play.
the one thing why its called the option is because the qb has the option of giving it to the fb or pulling it in and either running it or pitching it to the tailback.
this is how it works, pretty simple, this is out of a two back formation
when the ball is snapped
first the qb pre-reads the d-end,
then he sets up to hand the ball off to the fb, if the d-end comes down for the fb he pulls it out and either runs or pitch off the invert. If the D-end is "smart" and doesn't come down to tackle the fb the qb will give the ball to the fb.
