Are you talking about de-sync? If so, in Madden -- to put it as simply as possible, and it's similar enough in other games -- is that two people are more or less playing a game that is different in some way on each of their screens. If or when this happens in online games in general, you get "de-syncs" which can cause the game to become unplayable if it is not able to correct itself and make sure both users are having the same experience in ways that matter for stability.
As for why it occurs. I can't say for sure as I haven't read up on it enough, but I would assume it's because the host or server is pushing data to two or more people at a time. If something gets screwed up along the way, maybe via changing connection speed or whatever else, that can cause each user's game to become different.
