Equipment not showing up

Has anyone else noticed that some equipment that you put on a player not showing up? Or just showing up on its own? For example I put eye black on a player and he never has it on but one player I didn't put it on always has a nose strip and eye black. I don't get it

I haven't noticed an issue with eye black.

Is anyone else having a problem with cage, grid, 2 bar skill, and web face mask not being available in CFM?

They are available when roster editing outside of CFM. I've had this issue before and after patch.

2 months later and still not able to access these facemasks in CFM. Love the look of the grid/cage facemasks and would love to put them on my rookies. Anyone have any info on these?
