Sacks not counting?

I've seen this quite a few times, so I'm sure im not the only one to notice this. I've seen people complain about QBs "falling forward" when contacted to prevent a sack from happening, but what im experiencing is different.

I sack the QB, he loses 5 yards, but no sack is recorded. The announcers don't call it a sack, but they do mention the loss of yards. It is definitely a pass play, so I have no idea as to why this is happening. It costs my guys xp.

Does anyone know what triggers this?

EA is aware of the issue including tackles and sacks not being counted along with the falling forward issue.

Good news is they know about all 3 issues and are working to fix it.

So now the question becomes what else do they break in the process of fixing these issues.
