I had that option turned on. I think sometimes when I would turn the difficulty up when experimenting with sliders that it would turn it off and that's when the QB would throw the ball automatically.

I may have to try Coach mode this year..I always think about it but never do...Depends how the game plays this year

If I remember correctly it's in the controller settings menu.

Go to "My Madden",select "gameplay settings", set "auto-avoid" to "on".
