Need Help with MAdden

So I have had this issue a few times playing offline. My guy "fumbles" after being tackled, the replay shows he is on the ground and I go to challenge and nothing. Furthermore, when I challenge right away and look at their awful replays they show it is even clear he was down and yet the challenges are not successful? Is my game bugged? I find it really hard to believe a game could be considered "great" when it doesnt even get the rules of football correct. Is it my sliders? Is there another way to challenge a play? It seems really poor you cant challenge after seeing a replay.

Also I seem to be unable to throw balls deep on a streak route. I am sure this is my own doing but it seems no matter the distance I am alwayss 5 yards short and it gets picked off. I have tried using the total control passing pointing the joystick up feature to hit him in stride but like I said it doesnt matter if I am throwing it 30 yards or 50 yards its always short by 5 yards or so.

Challenges are a crap shoot. I never get them called my way

Are you throwing on the run? Who is your QB? Also, I never bother throwing those passes.
