The two scenarios I've noticed it happening in is when the goal is to strike out the hitter (which is a stupid goal to come from a manager anyway, but that's a different post) and when you're supposed to get ahead of the hitter.
The entire job of a pitcher is to get outs. There is simply no scenario where giving negative points when an out is recorded without a run scoring is in any way reasonable.
I really hope it's a bug, because I can live with those. But if it's a design decision, it really gets my goat because it goes against the realism that SCEA is aiming for with the game.
Seriously, can you ever imagine a manager saying, "Hey, getting him to ground out to short was nice and all, but I wanted you to strike him out, you failure!" I don't think so.
