I have Cespedes, Matz, Vargas, Dom Smith, Cano, Gagnon, Zamora, Bashlor, Rhame, Guillorme, Nimmo, etc.

It has to do with them getting sent down to the minors either during franchise or by editing the roster. It shouldn't be a hard fix for developers.

testing with some players movements I noticed that at least SDS roster there is no problem... using OSFM V1 it shows up fake portraits. thats very annoying.
