Given the way that other opposing pitcher/batter ratings play off of each other, I like to think that it's the same with clutch. In other words, both clutch ratings are compared when runners reach base and whichever is higher gets the advantage (weighted by how much of a difference there is between them). So if a pitcher and batter with the same clutch rating faced each other with runners on, there would essentially be no clutch effect for that at-bat.

Just theory, of course.
