I had the same issue with Fastbreaks. After looking into many sliders, I think I found a solution. After about 5 games, I'm averaging about 20 fast break points a game. ShadyMikeGaming's Movement, CPU Awareness, Ball Security, Body-Up Sensitivity, Strength, Speed, and Acceleration are paramount to Fastbreaks in my experience. Here are the sliders:

Movement:
Speed W/ball Max - 75
Speed W/Ball Min - 45

Acceleration W/Ball Max - 75
Acceleration W/Ball Min - 45

Speed w/out Ball Max - 69
Speed W/out Ball Min - 34

Acceleration W/out Ball Max - 69
Acceleration W/out Ball Min - 34

Offensive Awareness - 100
Defensive Awareness - 100

Speed - 50
Acceleration - 60
Ball Security - 0
Body Up Sensitivity - 75 (More Blow-by's and less Suction to nearest defender)
Strength - 50

This set up really showed the difference between fast players and slower players. Helped me generate more fast breaks. Hope this steers you in the right direction

Updated to v1.6. Got to spend some time with this during my day off yesterday and took it for a final spin today during lunch. I incorporated a LOT of what you all shared and suggested, so thank you for that. Really good stuff here.

I truly believe there is not a single desired result we cannot find within this game and slider set. If we're not there, we're very close.

Be sure to apply these updates from Superstar instead of All-Star. Not sure why that matters, but apparently it does. Doesn't hurt to be sure.

Here's a few clips of transition play and some turnovers. I didn't record anything until late in the game so these aren't even the best examples. Excuse the bad takes to the rim too I'm still learning this team. You'll see the box score at the end which I think is fantastic based on a lot of what we've been searching for here.

I’ve played 5 games with these and both me and my opponents are averaging by far the fewest shots in the league. Both around 75 with the next lowest being 81 and the league high being in the low 90s. I’m admittedly pretty new to this game and find the passing and plays to be a little clunky so maybe I’m just not running my offense fast enough. I’m just wondering what others are seeing. Do I just need to execute my offense quicker?

As a side note Lamelo Ball tied the nba record for steals in a game last game against the magic and also turned the ball over 4 times himself so we’re definitely getting plenty of turnovers and fast breaks.
