Myleague Online Forced Injuries?

Ever since the patch fixed in-game injuries for MLO, injuries are occurring whether the setting is turned off or on? I have even zeroed out the injury and injury severity sliders and still injuries are occurring. Is this a glitch or by design? MLO heads have been wanting in-game injuries for a while now and they are welcomed, but I am trying to figure out why they occurr even when the injury settings is turned off.

Also, for the offline ML guys, is there any rhyme or reason to how injuries occur or is it purely random. Is it driven by minutes played, durability ratings, or all of the above?

Any other MLO admins experiencing forced injuries this in their Online Leagues? This is not a game breaker as injuries can be reset or reduced by admins, but just curious by this design choice by 2k to force injuries regardless of the setting being on or off.
