It is a bit tricky to use at first, but once you get it it's very intuitive. The x axis is each individual minute and you select the five players for each minute. Don't worry about it putting the players in the wrong position at first. For instance, if you pick a point guard first to play a given minute, it will usually assign him the SG position by default. Ignore this and continue picking players. The system is pretty smart in assigning the correct players each position. If it messes up, though, in single-cell mode, press Y (I think it's triangle for PS4), and then swap the positions by selecting two players with A (circle I think).

Yes I use this all the time and the coach does his best job of keeping up with the timing and compensating when necessary

I've tried using it for under 12 min quarters but it never works correctly. So I gave up on it. This is why they need the clock to always start at 12 min for games and then adjust speed of the game clock to how many minutes we're playing like FIFA does. This way fatigue can be tied to actual game time and setting rotations would be 100x easier.

My main gripe with it really is that it doesn't accommodate foul trouble very well. I feel as though it is uncompromising in that regard. I often have to manually sub out players in-game, in spite of that horrible substitution menu.

A good thought, not the best idea since speeding up time will mess with the shot clock, which is absent in football.

Yeah that's the one thing that would be tough to translate. Maybe just keep it at the 24 seconds? that is the only flaw in my idea and i didn't even think of it. I feel like an idiot.
