How does this affect gameplay? Does it actually have an effect on the gameplay? Specifically if I'm simming cpu vs. cpu games? I see that there's a rating system, is it best to to keep the preferred coaching style, or aim for the set with the highest rating?

System proficiency is just a kind of artifical bonus/Malus system that is Not thought through very Well. they implemented in some years back, which was a Good idea back then, but then they forgot that it exists and didnt develop on that.How does this affect gameplay? This is for 2k18, FYI.
A recent test I ran showed no team made the playoffs with an overall system proficiency of less than three stars. A team with four star proficiency won the championship.
For simming it might have some impact.
As a user, you can probably overcome low proficiency but it’ll likely be easier with 3 stars and up.
However I think it makes sense to pick a system and build the pieces from a realistic standpoint from the coaching staff on down through the roster.
