The most realistic sliders I've come across are "The Purist(s)" but I get annoyed with realistic. The game becomes too hard to play. I would give those a go. Get online and search for The Purist sliders and download them. -

I would say it all depends on your definition of "realistic". Do you want realistic stats or do you want realistic gameplay? I prefer gameplay.Comment

I think for everyone there isn't really one slider set that is the most realistic because player's have their own skill level.Pre-NBA 2k12 V3.0 (360)

I want gameplay first and foremost. Stats are also very important to me but I want the game to feel like a real NBA game.
There are many good base set's here. Try them out (M.K Harsh, Purist etc) play at least 5-6 games with each. Then tweak to your style and skill.
Either way 2K made an amazing game that has years of replay value.Now Playing on PS5:

I agree with A & S... The problem with video games is that they have flaws. You cannot (IMO) get BOTH perfect stats and simulation play. I will take realistic play over stats because stats get skewed at times. For instance, the Bucks just got beat by the Celtics (for real) the other night in which Boston held the Bucks to the lowest point total in Celtics history. That's an abherration, but a possibility.
Now, in a video game that's the kind of extreme you want to avoid, so you immediately start changing sliders to prevent that. Again, it's a possibility that could happen.
Just play a number of games with a slider set (at least 5) and see what trends you find. If they work for you, great. If not, you've got plenty of options here...STEELERS INDIANS CELTICS

The slider set that I run with is my boy Makeufamous, to me his sliders just make sense. Here are two video from Youtube, the first is Makeufamous going through and explaining his sliders, the second one is from Dynasty1031 who put up video of gameplay using the slider set check it out.
