What makes an A+ visit?

You definitely should schedule on a game day. I've had recruits where I've checked off high activities during their visit, but its a bye week and I get a C+ rating.

I think scheduling a visit against a rival is much more key to getting the A+ visit than a visit against a good team.

As Michigan State, I scheduled some visits against Indiana (a rival, but a terrible team). IU was 0-6 when I played and beat them. All my visits that week were A+.

I then had some visits when I played undefeated and #3 ranked Iowa (not a rival). I beat them as well, but didnt have any visits above A-. And this was a much closer, more entertaining game.

In both cases I was making 3 pitches that were all High-Most and A to A+.

Wow, I really had no idea that you could only use one pitch and still get a good grade for a visit.
