QB sneaks?

I've found that nearly every time I try one on 4th and inches my QB just begins falling immediately and never picks up the yardage. It's not as if the DTs are getting a good push. The blocking can be perfect and it still doesn't work. Is this true for everyone else?

Yes. It has to do with whether or not the defense has 5 down linemen. If the D has 5 down linemen your QB will dive. If they have 4 or less, the QB should try a regular run. They did it because it used to be an automatic 2-3 yards every time, so I'm ok with it, but it should be improved. They need to continue to improve the physics of the linemen so pile ups work well and they can allow the QB sneak to possibly go for more yards again and be run normally.
