Rb Being auto subbed out and not coming back in

I play on a 7 min quarters and each time i get to the 4th quarter, my rb Ingram from Bama gets subbed out automatically and does not come back in. Its not injuries and this has happened to me in my online dynasty with demps from Florida. In the last game this happened with Ingram he had 17 carries for 110 yards, was taken out in the 3rd quarter (after a long run that was called back) and did not come back in. The game went to double overtime and I sat him out for half of the 2nd quarter in the hopes he would be in during the end of the game(to no avail).

Has this happened to anyone else, and is there a solution to fix this?

Its your player fatigue settings. Lets say the sub out is 80 and the sub in is 85 as the game goes on his fatigue drops. After its below 85 he won't come back any more because his sub in is 85 and he is below that in fatigue. So drop your RB sub in a bit lower and see if it works.

Are you sure your not winning big and it's mass subbing the guys out?

I've had this happen once in about 6+ years of dynasty. My running back left the game in the third and never came back. No injuries, no mass subs, nothing.

The fatigue setting thing makes sense perhaps, but does this mean that after a certain point in a game, it is impossible for a player to go above a certain numerical value? If so, that's awesome.

You are correct. When I was using custom sub settings this happened to me quite a bit, but it does not happen much, if at all with default settings. The problem with that is you do not get realistic split in carriers unless you continuously manually change your package.
