How many rarity levels of skill upgrades are there in RTG mode? And how much does the highest skill upgrade cost?

I haven't seen anything above a Level 5 and it was $34,500(ish).

I dont know the rarity level but when i was playing runningback there was an upgrade that came up. I forgot what it was called but it basically put every rating that is important to a runningback to 99.
It cost like 35000 points.
