You blitz this way by overloading the o-line. From a base nickel package, you blitz both linebackers resulting in either a lineback or a d-end getting into the backfield quickly. To decrease the time it takes to get to the QB, show your blitz pre-snap by making an adjustment to the defense. Also, from the same formation, there is a double safety blitz with a linebacker coming too. You have to leave the halfback uncovered. Blitzing like this will leave the HB uncovered on a screen, so pay attention and cover the HB yourself. The other guys on defense will be crashing the QB and get him. Just don't let the HB run after he catches the screen.
To counter this on offense, out of a two reciever set, start assigning your fullback and halfback to blocking assignments instead of running routes. Do the same to the tightend if you need that halfback to come out of the backfield. If the defense is constantly blitzing those backers, audible the tightend to a streak (go route) and hit him a soon as you snap the ball. You can also try shifting your linemans blocking toward a certain side.
What's important is that you slow your roll and try to read to the defense. If someone is getting to your QB as soon as the ball is snapped, then they're showing you the blitz preplay. Know that nearly all plays have your TE running a route, so think of the defender lined up over him as unblocked.
