Different Type of Injury Issue

I have since this issue three times so far. My defense is on the field and I'm playing as the Steelers. However, one of my offensive players receive an injury, when they are not even on the field since my defensive unit is there.

Is this an issue or is it possibly the fact the offensive player got injured on the previous drive but I just didn't get notified until the defense took the field?

Yes, your trainer notifies you a few plays after the injury with the news.

Maybe he got run over by the other team's Mascot in a golf cart (lol). I remember a few years ago my Saints were playing the Titans in an exhibition game and at halftime warming up the mascot actually ran over A. McPherson and that was it for him in a Saints uniform. You weren't playing the Titans were you.
