In the qualifiers, it depends on the region.

Asia has 4 groups, so the top 4 advance, and the best second place of all 4 groups plays the Oceania group winner.

CONCACAF has a group of 6 at the end, top 3 qualify automatically, 4th place plays against the South American 5th place.

I think Africa has the group leaders advance. Not sure Europe. South America obviously has the top 4 advancing automatically.

Once in the WC, Pielet would be correct in that the top 2 in each group advance.

Europe has 9 groups.

The winners of all 9 groups advance automatically. The best 8 second place teams are entered into a seeded playoff round, which sees the 4 winners of the home and home series' advancing.

after every world cup each region is assessed by fifa's own formula's etc and the number from each region will change based on the merit of the region. CONCACAF was expanded recently to allow more into the WC due to the US and Mexico's performances.
