You can hand a HB the ball 4-6 times in a drive before he needs a breather, unless he's taking a lot of abuse or breaks a long one, then it will be less. I haven't really seen WR's or OL sub out (maybe once or twice with a WR). I have a QB who can run (Troy Smith with Ohio St), and I've seen him get tired, but never have to come out of the game. On defense, my DL subs out regularly, and sometimes I'll see a LB take a breather as well.
