1. It's gotta be a volley and your positioning needs to be over the defender and at a higher spot than your head. I get most of these on corner kicks and crosses to the box. Depends on timing and luck really. Just use the shoot button.

2. If your guy doesn't have enough ball skills and passing rating it won't work. I've found that you hold LT and press A and Back on the left stick (relative to the player) at the same time. It doesn't work all the time for me tho. Just practice in the Arena for awhile.
