i try and keep this as close to default as possible, bc to me the game plays the best that way. might not be 100% realistic and these might not be for everyone, but give them a try and adjust to your liking.
if any slider changes i make dont sit right like they did in Madden 12, i will then adjust in increments of 25. i also at times use the in between levels (ex 37 being in between 50/25). making the adjustments that way again keeps things the way Madden adjusts levels by default.

if you cant find a happy medium when using say 50 or 25 for WR Catching for example. 50 too many catches, 25 too many drops, go ahead and use 37 (the in between number), and i bet you will be pleased. that goes for any of the numbers above, not happy with the 25 point increment, look at the in between scale. with having only 9 options to set your sliders, a lot less trial and error and more playing.
slideritus wont run wild if you stick to this default set up...

game assists, in the game options are just a personal preference, up to each user to use what they want for those.
fatigue, at 62 or higher from my testing the starters (RB) disappear in the 4th quarter. i then tried it at 55 and seemed to work well, back ups got a few carries. i went with 50, but if you want to move it up, dont go past 55 IMO.
speed threshold, 50 or 100, again this is a user preference. i thought the higher thres would make special teams harder, it didnt. i noticed ZERO difference between 50/62/75/87. i did however notice a difference in the game when using 100. nothing that fixes special teams, but the overall feel of the game on offense and defense i really liked.
quarter length, to get around 130 plays on average, 13 minutes is possibly the way to go. depending how fast you call plays and where you have min play clock set at. i personally like 12 min quarters and the results i get. usually 113 to 120 plays, thats good enough for me.
injuries, i used 50 and 25. way too many injuries with both of those settings. i settled on 12 and still seem a decent amount of them.
