Or did he get re-injured because he had already gone out with a "medium" risk injury? There really needs to be better documentation from EA about the mechanics of injuries, especially mid-game injuries and substitutions...

Sounds like it was a minor injury. Some injuries in the game the player will only be out a quarter or two. You can still play him but he won't play as good and his stamina will be so low you risk another injury. Once his injury is over (usually 2 quarters) he will return to his normal spot.
