According to Markus, literally the only thing is no drug suspensions in MLB games. Nothing else is affected.

I know in previous games you could limit your league by Nationality so that Japanese leagues have majority Japanese players, but can you also do this for race if I wanted to setup the Negro Leagues?

I have a feeling that through mods we'll be able to add things like PED suspensions back in. I could be wrong though.
